4. Use the following information for "who would you like to receive the acknowledgment?" (Step 1b)

5. The next part is "From." Enter your own name.
6. Next is "Personalized Message." Please write your own message, (limited to 255 characters or less) but refrain from using any symbols in the message or anywhere in the form or your card will not print correctly. Here’s an example of a message: (Step 1c)
"Happy Birthday Zachary! Thank you so much for bringing so much joy to the lives of millions. May your birthday be grand and may this gift serve others as well as you have your fans. Sincerely,"
(Be sure to click on "Preview Acknowledgment" so you can correct any mistakes before moving on.)
7. Click the "next page" button which is where you will "add a donation amount" and your "payment method" (credit card or electronic check). You can donate as much as your heart desires, but a minimum of $5 is suggested. (Step 2)
PLEASE NOTE: There is a drop-down menu in the billing info section where you must choose your country. If you are from outside the U.S. and are still having problems donating, you can handwrite all the info required in the online forms and send a check or money order to:

In acknowledgment of your donation, The American Heart Association will send a card (see example HERE) to Zac’s publicist, who will make sure Zac gets every one. (And she says that he reads every card and letter sent to him.)
